RN Case Manager Job Type: Hospice Case Manager

Required Education: Licensed in the State of Texas
Required Experience: One year experience as a professional nurse preferred. Ability to work in a field setting and exhibit ability to make sound nursing judgments. Small case loads, will provide training on all hospice nursing services. Positive team environment. Ability to assess patient needs and formulate individualized patient care plans to meet those needs. Maintain accurate detailed reports and records. Monitor, record and report symptoms or changes in patient’s conditions. Computer skills required. Must be able to communicate effectively with the Interdisciplinary Team.
Required Travel: Greater Houston and Surrounding Areas

Many opportunities are available. Patient and family companions, weekly “check-in” calls, making comfort blankets, office helpers, light house chores, shopping, and yard work are just a few of the ways you can serve the families of Gulf Coast Hospice.

All volunteers are guided through a comprehensive training .
